Electronic Health Record (EHR)

Q: Who can I contact if I have a general question about using the Electronic Health Record?
A: You can contact the How-To-Helpline at 252-744-2065 and request to speak with the trainer on-call. You can also contact your designated department trainer.

Q: I forgot my EHR password or I want to change it. Who can I contact to get this corrected?
A: Call the ECUHealth Help Desk at 252-847-5111.

Q: How soon will I receive my access?
A: Once the Team Dynamix ticket is submitted by the Hiring Manager, please allow 7 to 10 working days for the processing of all requests. Employees will receive their User ID and Password through their ECU Email Account.

General Medical Records

Q: How can a patient acquire a copy of their medical record?
A: Patients are authorized to obtain copies of their medical records from ECU Physicians in most cases. They may contact the Medical Records Office at 252-744-2065 for more information. There are exceptions, such as a provider determination that access or copies of these may be detrimental to the physical and/or mental well-being of the patient. Absent this, a patient needs to simply complete an Authorization for Use/Disclosure of PHI (PDF) and forward this to the Release of Information Section/Brody Bldg., fax to 252-816-0081, email to hissroi@ecu.edu, or mail to:

Centricity

Q: How can I go in and view old medical records that were kept within Centricity?
A: Old medical records that were recorded in Centricity can be retrieved by accessing the Centricity Data Warehouse webpage. Once on this page, users will input their Pirate ID and Password. They will then click Apps at the top of the screen, and then click the Centricity – Medical Data Warehouse Link. Once this page loads, users can then access those medical records found on Centricity. If you are unable to login to the Data Warehouse or cannot access certain features on the page, call 252-744-2030, select option 2, and let the individual know you are having trouble accessing the Centricity Data Warehouse.
